Hi, y'all! I am Nurse Sarah! ♥

I live in Ovilla with my husband of 8 years and our 3 kiddos; Jack-6, Luke-4, and Kate- 5 months old. My home life is chaotic and busy, but I wouldn't change it for anything! I graduated from Tarleton State University with my BSN in 2015. After graduating, I worked at Parkland Hospital in Dallas in the Neonatal ICU for 6.5 years before switching to the school setting. This will be my 3rd year here at ROMS and I love it! It has been so rewarding to watch the students grow and mature over their 3 years in middle school.

I have included some of the common required forms that students might need throughout the school year. I can only keep prescription medication or over-the-counter medication with a doctor's signature on the form. Action Plans/Diet Modification/Permission to Carry forms must be signed by a doctor and have to be resubmitted yearly. Per the State of Texas, all incoming 7th graders must receive an updated TDAP and Meninigitis vaccination to begin 7th grade. I attached some clinics that are able to give vaccinations to students.
